Background Record Check

A "Background Record Check" is an itemized list of arrests, with a disposition when available for each arrest, verifying an individual's record within the City of Waukesha. This is usually done for prospective employers, passport/visa purposes, or adoptive agencies.

To make a Background Record Check Request click on the Background Record Check Form and fill it out.

Cost: $10.00

Payment:

  • Mail: check to 1901 Delafield St. Waukesha, WI 53188.
  • Drop-box: drop payment off in a sealed envelope at the drop box located at 130 Delafield St. Waukesha, WI 53188. The green drop box is on the front of the building. Check or cash will be accepted. Write on the envelope (name, phone number, what payment is reference)

Completion Time: Approximately 5 - 7 working days to complete. Once complete and payment is received the document will be emailed to you if you provided an email address. If you did not provide one, a member from the department will contact you, at the provided phone number, to make arrangements to schedule a pick-up appointment. You must talk to a person at the Police Department to arrange pick-up. No one will be available at the department for in-person inquiries of this nature and you will be turned away.

Information Needed to fill out link above:

  • Full name of the person, including middle initial
  • Date of Birth
  • Sex and Race
  • A Driver's License or photo I.D at time of request
